This little guy was so much fun in my consulting room this morning! I am used to the bubs and toddlers exploring my room and getting into mischief, but this little guy was exploring things differently, e.g. he was literally was upside down in my toy box with legs in the air (I wasn’t quick enough to get that photo!) and he then wanted to sit on my lap for his heart and chest stethoscope examination, so sweet!

These types of baby and infant consults make my day and constantly remind me why I love my job!

There are many reasons why many women do not enjoy being pregnant. There is enormous pressure to feel wonderful and happy during their pregnancy, and many people are too scared to admit they are struggling.

It may be they feel sick or uncomfortable, have had a long and stressful fertility journey and can’t relax until bub is born, they might be anxious about the birth & the future, have trauma from a previous experience, feel out of control, or maybe they are struggling with changing body shape and image, the list goes on.

Know that this is common and normal for so many women. You are not alone. Women need to talk more about this, stop pretending it’s all rosy and that they should be glowing. You’ve got this! And remember to connect and reach out to family, friends and health professionals for support if needed.

We go through antenatal expressing if that is of interest, and also cover what not to do!

After birth, I encourage my mums and bubs to come and see me as soon as possible after discharge, to troubleshoot any issues and nip any breastfeeding problems in the bud. I am also a huge fan of undoing and breaking the triple feeding nightmare that so many people experience after discharge and in those early days!

My breastfeeding consults are for. 60-75 min, tailored to you and your individual circumstances.

✨Dont wait. The time will never be just right✨

Whether it be trying for a baby, applying for a new job, deciding to travel abroad, starting a new training course, getting a puppy, learning a new skill or hobby, reconnecting with a friend or loved one…. the list is endless.

We need to practice living more in the present, take opportunities when they arise, know that there is no such thing as perfection or a perfect time for many things, be brave enough to make those hard decisions and then run with them, to wherever they may lead us.

It’s time to explain why so many of these strategies aren’t helpful, lead to a shallow latch that causes nipple trauma and make your pain and discomfort worse!

5 DON’Ts:

This little cutie and her mum came to visit me yesterday at MIWM in Parkville. It’s the best job ever, supporting new parents and their little ones during that crazy time we call the perinatal period.

Little baby Lilly is such a cutie, the GP examination couch is her favourite place.

My main problem is that little bubs like Lilly are so sweet and give me such heart warming smiles while I am consulting, that I am completely distracted by their charm and lose my train of thought.

But I love it!

Is your nipple and breast pain really due to nipple thrush?

The short answer? Rarely.

Yes there are treatment protocols, yes things often do get better (eventually) with treatment after long drawn out courses of a antifungal medications, but the reality is, pain that is commonly thought to be thrush is usually due to microtrauma, caused by a suboptimal latch.

I love providing breastfeeding consults to help women get a close, pain free latch. And then what happens? The pain, inflammation and trauma usually settles down and goes away.

I really enjoy providing shared care. I enjoy providing this continuity of care and find it really rewarding, supporting & caring for patients physically & emotionally, all the way through their perinatal journey, from fertility, through conception, pregnancy and then in the postpartum.

Most antenatal appointments are with the accredited shared care provider, with only 3 or 4 appointments being at the hospital. If any complications arise then the hospital will usually take over your care.

🍼 Paced bottle feeding🍼 If you use a bottle to feed your baby expressed milk or formula and you have never heard of this, then this video is worth watching!

Paced bottle feeding is a technique of bottle feeding a baby that more closely mimics breastfeeding, encourages the baby to work a little harder to suck the milk out as would happen on the breast, slow the feed down (pacing it) and enables the baby to be more in control of the feed, show cues when they are hungry, want more milk or if they have had enough.

I will often spend time with my mums and bubs during my consults showing them this technique, but it is especially important for the dads and other parents, grandparents or carers to know too!

Our understanding of the mastitis process has changed over the last 1-2 years. These 3 pieces of advice are old and outdated, and are often making your symptoms worse.

There is a lot more that I go through to help mums and bubs get the right latch (fit &h old) when they come to me for breastfeeding support, but I feel this closeness is definitely the most important cause of breastfeeding problems.

We do have general recommendations that women under 35 with no obvious issues should try for 12 months before being investigated for infertility, and those over 35 should try for 6 months, but this advice is very general and may not be the best advice for you and your personal circumstances.

If you are not sure, book with your GP to discuss. Even better, make an appointment for a pregnancy planning consultation, so that the advice you get is tailored to you and your own circumstances and you know what to do and expect before you start trying. A pre-pregnancy consult is also invaluable for many other reasons, check out my other Perinatal Pearl #5 for more info!

The six week mum and baby checkup…. What is it, and do I need to book one?

Anyone know the feeling? Baby is born, you get sent home from hospital with your new baby, have no idea what you are doing and they tell you to go and see your GP in 6 weeks for a checkup. 6 weeks is a very long time!!

A six week check is really a formal name for a postnatal doctor checkup for both mum and baby.

Seeing your GP, obgyn or paed for a six week checkup is a really good idea. Us medical people have lots of things we like to check for to ensure you and bub are healthy, are recovering well from birth and are both thriving.

Does it have to be at 6 weeks? This is a suggested time frame, but I honestly believe mum & bubs should have this type of checkup much earlier, before little issues become big issues.

In my consults, I easily spend an hour+ with mum and bub, as I try to cover so much in this appointment, not only checking on your physical and emotional wellbeing, your birth recovery, troubleshooting issues, talking contraception, feeding, pelvic floor, but also checking that baby is thriving. I like to address all concerns, whether they be big or small, answer any questions, give general parenting advice, vaccine advice, what to look out for if you feel baby is unwell, the list goes on!

In my consults, I easily spend an hour+ with mum and bub, as I try to cover so much in this appointment, not only checking on your physical and emotional wellbeing, your birth recovery, troubleshooting issues, talking contraception, feeding, pelvic floor, but also checking that baby is thriving. I like to address all concerns, whether they be big or small, answer any questions, give general parenting advice, vaccine advice, what to look out for if you feel baby is unwell, the list goes on!

Mastitis treatment - New(ish) guidelines you ✨NEED TO KNOW!✨

Have you been told to massage your breast to help mastitis and blocked ducts? Increase feeds to remove or empty milk from that side? Use heat or a warm shower to help the milk flow?
⚡️WRONG!!!⚡️
This old advice is usually making things worse.

Understanding that mastitis is an inflammation of breast tissue and the duct itself which narrows down to block flow, rather than a “blockage” in a duct, is vitally important as it can mean the difference between a successful breastfeeding journey and a nightmarish one!

Inflammation (swelling, redness, tenderness, fever and feeling yuck) can be caused by this inflammatory process, and is often not a bacterial infection.

Please note, there is absolutely a role for antibiotics in certain circumstances. There is also a type of massage called lymphatic drainage which may help, but the first step is being informed.

Perinatal Pearl #5: When should I see a GP if I am thinking about having a baby?

In short, the sooner the better!

A pregnancy planning consultation is always worthwhile, I like to discuss:
Optimising your health
Finding out about your fertility concerns and ensuring there are no issues that need to be investigated
Blood tests to check your general health and also immune status against vaccine preventable diseases that can be dangerous in pregnancy
Ensure your CST (pap smear) is up to date

Discuss optimal genetic testing that is available
Discuss your age, how late to leave it?
Discuss various fertility options including fertility preservation, egg freezing
Discuss what vitamins/supplements to take and address other lifestyle concerns
When is your fertile window, how to track cycles, when to have sex, how long to try for before you may need further investigation or referral….

As you can see, the sooner the better! And don’t leave it too late! Fertility and egg quality starts taking a nosedive for women from age 35.
